Enclosed are copies of my insurance claim. On April 10th, 2017, Humana informed me they will not pay any of my medical bills, in regards to the above claim number. I have worked for Al Estes Bonding Agency, for 12twelve years, and I have my health insurance through my employer.

The reason Humana gave, was Tthat I have an insurance policy with United Health Care from 2004. I spoke with United Health care and they have an active Health Insurance policy oin my name from 2004. They confirmed they have not paid any medical bills since 2004, and have not mailed any insurance cards. I believe United Health care has an error in their computer. I am concerned, as I need these medical bills paid.
